I don't know if anyone has asked this yet (of course it has been brought before), but, during the game play, can you change your kingdom name and flag?? or it only happens when you mod. it???

you can change your kingdom name and flag, but only in some pre-defined cases.
You can't do that whenevery you want, but only in specific cases when you unite certain kingdom

I'm Leinster on early medieval, i conquered all ireland, wales and scotland + hal england.... could I change my kingddom name from Leinster to Ireland?

only if you do all what is necessary for game quest of unitingf ireland, that means, you must conquer irish lands and eliminate all other irish earldoms. If there was no Munster and Ulster you would already be Ireland.
But you can't do it when you decide. Game has pre-defined quests (which you may or may not know)and only if you complete them you will get an offer to change name and flag.

So f.i. you are able to rename to Ireland if you conquer irish lands with Leinster but you won't if you conquer Ireland with say Scotland or Normandy
