when i start the game, i've only a reign or some reigns?

For example: i start in Italy: i have only Genova, or some other italian reign, near Genova, like Milan or Toscana?

When you start, are there any provinces they arent protected by anybody?

All realms belongs to someone. There is no "barbarian" territories, "gray" territories, "non-playable" factions, "minor" kingdoms etc. fiction :)
